At our next meeting on 21 apr 09 we will be collecting fishing shirts to get our names and logos embroidered on them. The cost will be $8 per shirt embroidered. But you provide the shirt! Please download and print the order sheet (it looks something like this). Fill it out and put in a bag to turn-in with your shirt(s) Click the link to the pdf order sheet.

Please fill this out completely (make 2 copies) 1 goes in the bag with your shirt and PACK will hold on to the other. Name and Logo embroidery is $8 per shirt – prepaid. Checks made out to PACK.
